We now have a consulting Pain Management Physician, Dr. James Sturm that will be working with us on limited days a month, (first day is January 31st) helping patients get signed up for Medical Marijuana Cards. They also sell a variety of excellent CBD products such as sub-lingual oils, gummie treats, topicals and even chocolate chip cookies.
If you are interested in just CBD products he has specially trained staff that can help determine what would be best for your medical condition.
Payment must be made at the time of service, $150 for consult and forms for Medical Marijuana Cards (you would then need to register yourself online). If you are not so computer savvy and would like them to submit the application on your behalf it is an additional $45. Appointments are 30-45 minutes and you must be on time with all required documents. For a consult for CBD products only is $45. This does not include the price of selected CBD product.
Below are medical conditions which qualify for a Medical Marijuana Card:
Intractable migraines unresponsive to other treatment
A chronic medical condition that causes severe, persistent pain or persistent muscle spasms, including but not limited to those associated with multiple sclerosis, seizures, Parkinson’s disease, and Tourette’s syndrome
Debilitating psychiatric disorders, including, but not limited to, post-traumatic stress order, if diagnosed by a state licensed psychiatrist
Human immunodeficiency virus or acquired immune deficiency syndrome
A chronic medical condition that is normally treated with a prescription medications that could lead to physical or psychological dependence, when a physician determines that medical use of marijuana could be effective in treating that condition and would serve as a safer alternative to the prescription medication
A terminal illness
In the professional judgment of a physician, any other chronic, debilitating or other medical condition, including, but not limited to, hepatitis C
Amyotrophic lateral sclerosis, inflammatory bowel disease, Crohn’s disease, Huntington’s disease, autism, neuropathies, sickle cell anemia, agitation of Alzheimer’s disease, cachexia, and wasting syndrome.
The following information and documents will be needed at time of visit.
1. Proof of Missouri Residency (one of the following)
Copy of a valid Missouri Driver’s License
Missouri Identification Card
Current Missouri Motor Vehicle Registration
Recent Missouri Utility Bill
2. Physician Certification Form, you may obtain one at: https://health.mo.gov/safety/medical-marijuana/how-to-apply-pi.php
As part of the application process, Patients must obtain a Physician Certification that is issued
no later than thirty days prior to the time patient submits their application.
Patients should download the Physician Certification from the Department’s website and take it
with them to their medical examination.
A Missouri-licensed physician, active and in good standing, is required to complete and sign the

3. Additional Required Items
A legible copy of the qualifying patient’s photo identification issued by a state or federal
A Parental/Legal Guardian Consent Form for non-emancipated qualifying patient.
A clear, color photo of the qualifying patient’s face taken within the prior three (3) months.
